How to remove Lip Nodules from fillers such as Restylane ,Juvederm, and Silicone.

When dealing with nodules in the lip it is important to know what material was used to augment the lips. The treatment really varies depending on what was used and how long ago the treatment was performed. Occasional Hyaluronic acid compounds (Restylane, Juvederm, Volbella, etc) can ball up and cause small bumps. These usually resolve in a couple of weeks with massage or dilute injection of a steroid anti-inflammatory solution called kenelog.  Another option is to dissolve the Hyaluronic acid completely with a protein called Hyaluronidase. This material is injected directly into the affected area. Results are quite impressive and visible within 2-3 days.

Another condition that is much more difficult to treat is called 'Bio-film". We don't exactly understand why some patients basically develop a hard film around the implants in their lips/face! These nodules are also injected with Kenelog steroid, treated with Oral antibiotics, and anti-metabolites such as 5FU. If several injections over a few weeks does not dissolve the nodules they can be excised.

Finally; non FDA products such as Silicone and Bioalcamid (seen more frequently in patients from Mexico and South America) don't respond well to steroid injections and can cause severe deformities. These "granulomas" need to be excised. There is always scarring and downtime with excision of these inflammatory nodules in the lip and I perform it as a last resort. The incision is placed inside the mucosa and the hard nodules are surgically removed. Downtime and swelling/bruising is about one week.

Call us if you have irregular bumps or nodules in your lip or face and wish an evaluation to have these lesions removed.

Dr. A. David Rahimi, lead surgeon at Forever Young in Los Angeles, has seen a dramatic increase in male patients in less than twenty years. “When I finished my training in 1999, maybe 5 percent of my patients were men,” he says. “Now, one-third of my patients are men.” Advances in surgical and nonsurgical techniques have made cosmetic enhancements more readily acceptable. “Twenty years ago, all we had available was cutting surgery,” he notes. “We now have many, many options to tighten up the skin, remove brown spots and fine lines, and make someone look healthier and younger.”

Dr. Rahimi, on the other hand, has a more conservative outlook. “More and more men will speak about receiving cosmetic surgery, but it’s still taboo because, unfortunately, some practitioners go overboard. They do a little and it looks good, so the patient asks for more. The [surgeon] agrees and they make the client look done or fake,” says Dr. Rahimi. “And I don’t think men are going to talk at parties with each other about getting Botox or fillers,” he adds. “It’ll always be something that they do but not readily admit to—but it’s becoming more acceptable in our society.”

venus Viva for the non surgical tightening of the skin

The non ablative Diamond hand piece of venus Viva is ideal for tightening of the skin. There is no downtime and no pain while getting the treatment.

Venus Viva also has an ablative hand piece that inserts 36 small needles under the skin. RF energy is transmitted through the needles and tightens the skin. This modality does have a 3 day downtime (swelling and peeling).

Many conditions ranging from Rosacea to acne scarring and discolorations can be treated with these 2 types of Venus Viva. Please watch our video and consider seeing Dr David Rahimi for a complimentary consultation.

Bellafill for the permanent correction of nasolabial folds

I have now used Bellafill for the past 3 years. I have tested nearly a 100 patients and seen no adverse reactions to the test spots. Allergic reactions to the Collagen portion of Bellafill are possible but very rare.

Overall I have to say that I am very impressed with Bellafill. Besides the rare occurrence of bruising and swelling I have only seen deeper nodules twice. Both times the "granulomas" resolved with massage and dilute kenelog injection (kenelog 2.5mg/cc 0..2 cc).

I have found Bellafill to be effective for deeper rolling acne scars. I perform subcision with a NoKor needle followed by injection of Bellafill at various levels of the dermis. Deeper laugh lines can also be filled with Bellafill. This is an FDA approved indication.

As an off label use I use Bellafill to augment cheeks and the chin. Deeper layers of Bellafill can also be placed judiciously along the jawline. This results in a lifting of the cheeks and sharper and more well defined demarcation between the cheek and neck. It is ideal for patients who have lost a lot of weight or are experiencing sagging from excessive sun exposure.

Thermi vs. Ultherapy, vs. Pelleve, vs. Thermage CPT -Which one is better?

I am often asked to explain the differences between these 4 main non-surgical technologies.

Educated patients are often confused after reading about all the possible options. Which one is better is a complicated question and requires a little bit of explanation.

Allow me to ask you a question before delving into explaining the 4 modalities (5 if you want to include venus Viva).

Which medication is stronger for a headache; Tylenol or Advil or Excedrin? The answer is that some patients respond better to Tylenol and others have a more immediate response with Advil.

This is Dr. David Rahimi's 20th year practicing Cosmetic Surgery and Dermatology and our center own 30 different laser and light sources. We have it and perform it If it works well and is safe.

Ultherapy and Thermage CPT work on a deeper layers and are more aggressive. Fewer sessions are needed and results are usually more impressive and long lasting. Venus Viva, Pelleve and Thermi are usually more conservative but require more sessions.

Choose a board certified Dermatologist with experience and after a face to face interview he or she can tell you which one is best suited for you.

Consultations are complimentary and we offer Saturday appointments and procedures.

How are Thyroid and thyroid cancer scars removed?

I have seen and treated many patients with unsightly and thickened thyroid surgery scars. The scar itself can be treated with Diolite laser for the removal of redness and Mixto Co2 laser for the removal of the thickening and hypertrophic parts. Furthermore the loose skin around the neck can be safely treated with Ultherapy (Ultrasound neck and face lift) or Thermacool CPT (Radiofrequency face and neck lift). Please see us for a complimentary consultation.

The ideal time to have these procedures performed is debatable. I tell my patients that it is never too late but that best results are achieved if the laser and tightening is performed in the first 12 months after the recovery.
