I have now used Bellafill for the past 3 years. I have tested nearly a 100 patients and seen no adverse reactions to the test spots. Allergic reactions to the Collagen portion of Bellafill are possible but very rare. 

Overall I have to say that I am very impressed with Bellafill. Besides the rare occurrence of bruising and swelling I have only seen deeper nodules twice. Both times the "granulomas" resolved with massage and dilute kenelog injection (kenelog 2.5mg/cc 0..2 cc).

I have found Bellafill to be effective for deeper rolling acne scars. I perform subcision with a NoKor needle followed by injection of Bellafill at various levels of the dermis. Deeper laugh lines can also be filled with Bellafill. This is an FDA approved indication.

As an off label use I use Bellafill to augment cheeks and the chin. Deeper layers of Bellafill can also be placed judiciously along the jawline. This results in a lifting of the cheeks and sharper and more well defined demarcation between the cheek and neck. It is ideal for patients who have lost a lot of weight or are experiencing sagging from excessive sun exposure.

I am often asked to explain the differences between these 4 main non-surgical technologies.

Educated patients are often confused after reading about all the possible options. Which one is better is a complicated question and requires a little bit of explanation.

Allow me to ask you a question before delving into explaining the 4 modalities (5 if you want to include venus Viva).

Which medication is stronger for a headache; Tylenol or Advil or Excedrin? The answer is that some patients respond better to Tylenol and others have a more immediate response with Advil.

This is Dr. David Rahimi's 20th year practicing Cosmetic Surgery and Dermatology and our center own 30 different laser and light sources. We have it and perform it If it works well and is safe.

Ultherapy and Thermage CPT work on a deeper layers and are more aggressive. Fewer sessions are needed and results are usually more impressive and long lasting. Venus Viva, Pelleve and Thermi are usually more conservative but require more sessions.

I have seen and treated many patients with unsightly and thickened thyroid surgery scars. The scar itself can be treated with Diolite laser for the removal of redness and Mixto Co2 laser for the removal of the thickening and hypertrophic parts. Furthermore the loose skin around the neck can be safely treated with Ultherapy (Ultrasound neck and face lift) or Thermacool CPT (Radiofrequency face and neck lift). The ideal time to have these procedures performed is debatable. I tell my patients that it is never too late but that best results are achieved if the laser and tightening is performed in the first 12 months after the recovery.

Many of my patients notice aging of their eyelids starting in their 40s and early 50s.

We do offer cutting surgery and removal of skin and fat but this option is only acceptable to a small percentage of my patients.

Cutting surgery of excess skin and fat (Blepharoplasty) requires downtime and is associated with scarring.

Rarely scars can become thickened and unsightly. There is also the usually larger cost of cutting surgery and downtime.

Non surgical treatment of the eyelids with Restylane fillers, Lasers such as Mixto Co2 laser

and Ultherepay are alternatives that should be considered.

Excess skin can be tightened and removed with laser. Hollowness and deep "tear troughs"

can be filled in with fillers such as Restylane and Juvederm. Pelleve, Venus Viva,

Thermacool CPT, Ultherapy are non surgical laser, Ultrasound and radiofrequency

devices that can tighten and lift the skin without the need for cutting surgery.

Downtime is usually shorter and the procedures are generally safer.

One of the biggest challenges in treating patients with darker skin color and acne scarring is the possibility of blotchy skin after the treatment. This is called post inflammatory hyper pigmentation. Basically patients with an olive skin complexion (Asian, Hispanic, Ethnic skin) can develop blotchy skin and discoloration after laser treatment.

I often per-treat my patient's skin with a fading lotion (hydroquinone cream) and Retin A 1-2 weeks prior to aggressive laser treatment. Another more recent advance has been the introduction of venus Viva nono-fractional radiofrequency treatment. This incredible device is "color blind" and is ideal for the treatment of acne scarring in darker skin colors. Another advantage is that skin heals in 2-3 days (rather than the 5-10 days it takes for aggressive Mixto CO2 lasers).

I often combine the procedure with Subcision, dermal fillers such as Bellafill and Sculptra, and even laser resurfacing. Please read out this on our Acne site

Acne scarring can be both physically and emotionally very traumatic. I have dedicated the last 19 years of my professional life to helping patients with unsightly and often disabling acne scarring. No 2 people are the same and no 2 scars on the same face are exactly the same. The key to being successful and achieving incredible results is to analyses each scar separately and treat it using the best possible options out there.

I was very fortunate to train with a brilliant physician and surgeon with the name of James Fulton, III, MD.

He was eccentric and aggressive but kind and caring. Among his accomplishments was the co-development of Retin A in the 1960 with Dr. Kligman. He also developed Benzamycin topical antibiotic solution for the treatment of acne. In the 1980s he had many acne clinics all over Los Angele and treated hundreds of young adults.

Since 1999 I have improved on Dr. Fulton’s approach to acne and acne scarring. New laser and radiofrequency devices have make acne scarring treatments safer and more affordable.

Kybella for the treatment of excess fat under the chin and neck is becoming more and more popular.

The fact is that men in 2016 have several options to improve their jawline and contour their neck. In fact, 73 percent of people say they don’t like the extra fat under their chin and neck, according to a survey by the American Society of Dermatological Surgery found.

Here are the surgical and non-surgical options:

Smartlipo Triplex uses 3 different wavelengths and a laser to gently remove excess fat and tighten the skin from within. This procedure is invasive but can be performed under local anesthesia and leaves practically no scarring. Downtime is 3-5 days and cost varies between $2500-$5000
