Is Accutane still available? Is it safe? What is the difference between Absorbica, Accutane, Isotretinion, Myorisan, Amnesteen, Clavaris
I have practiced Dermatology and Cosmetic surgery since 1995. I have treated several hundred patients with Isotretinion and its generic equivalents. I can tell you without any doubt that this is one of the best and most effective medications on the market today.
Accutane is derived from Vitamin A and shrinks the sebaceous glands and pores and can significantly reduce acne. Ninety percent of my patients never have acne again. 10% need a second a rarely a third course. Unfortunately this medication is widely misunderstood. A very small percentage of patients can have side effects ranging from dry eyes, and dry skin to mood disorders such as depression of inflammation of the digestive tract. Most of these side effects are mild and they do resolve after stopping the medication. It breaks my hard to see a patient experiencing deep cystic and scarring acne who is afraid to even try low dose Accutane.
My advice is this: find a good dermatologist, make sure it is right for you and try a course of it.
At any given point I have between 20 and 50 patients on Accutane.

I have now used Bellafill for the past 3 years. I have tested nearly a 100 patients and seen no adverse reactions to the test spots. Allergic reactions to the Collagen portion of Bellafill are possible but very rare.
Overall I have to say that I am very impressed with Bellafill. Besides the rare occurrence of bruising and swelling I have only seen deeper nodules twice. Both times the "granulomas" resolved with massage and dilute kenelog injection (kenelog 2.5mg/cc 0..2 cc).
I have found Bellafill to be effective for deeper rolling acne scars. I perform subcision with a NoKor needle followed by injection of Bellafill at various levels of the dermis. Deeper laugh lines can also be filled with Bellafill. This is an FDA approved indication.
As an off label use I use Bellafill to augment cheeks and the chin. Deeper layers of Bellafill can also be placed judiciously along the jawline. This results in a lifting of the cheeks and sharper and more well defined demarcation between the cheek and neck. It is ideal for patients who have lost a lot of weight or are experiencing sagging from excessive sun exposure.

One of the biggest challenges in treating patients with darker skin color and acne scarring is the possibility of blotchy skin after the treatment. This is called post inflammatory hyper pigmentation. Basically patients with an olive skin complexion (Asian, Hispanic, Ethnic skin) can develop blotchy skin and discoloration after laser treatment.
I often per-treat my patient's skin with a fading lotion (hydroquinone cream) and Retin A 1-2 weeks prior to aggressive laser treatment. Another more recent advance has been the introduction of venus Viva nono-fractional radiofrequency treatment. This incredible device is "color blind" and is ideal for the treatment of acne scarring in darker skin colors. Another advantage is that skin heals in 2-3 days (rather than the 5-10 days it takes for aggressive Mixto CO2 lasers).
I often combine the procedure with Subcision, dermal fillers such as Bellafill and Sculptra, and even laser resurfacing. Please read out this on our Acne site

Acne scarring can be both physically and emotionally very traumatic. I have dedicated the last 19 years of my professional life to helping patients with unsightly and often disabling acne scarring. No 2 people are the same and no 2 scars on the same face are exactly the same. The key to being successful and achieving incredible results is to analyses each scar separately and treat it using the best possible options out there.

I was very fortunate to train with a brilliant physician and surgeon with the name of James Fulton, III, MD.
He was eccentric and aggressive but kind and caring. Among his accomplishments was the co-development of Retin A in the 1960 with Dr. Kligman. He also developed Benzamycin topical antibiotic solution for the treatment of acne. In the 1980s he had many acne clinics all over Los Angele and treated hundreds of young adults.
Since 1999 I have improved on Dr. Fulton’s approach to acne and acne scarring. New laser and radiofrequency devices have make acne scarring treatments safer and more affordable.

One of the most important lessons I have learned after treating Acne and Acne scarring over the past 20 years is as follows:
No single treatment works for everyone.
Acne has many causes including hormonal, bacterial and genetic components.
Acne scars also come in a variety of form and shapes: icepick, box-car (a misnomer), atrophic, rolling are just a few examples.
My approach has always been to reduce or cure acne and attack the scars using a variety of treatment modalities.

How to treat Asian skin with Acne Scarring:
Patients with Asian skin have more delicate skin and require a different approach when it comes to acne scar removal.
Mixto fractional Co2 laser is a fractionated CO2 laser that can be safely used in darker skin types. We still prefer to prep the skin for 10-14 days with a combination of Retin-A cream and Hydroquinone. Laser setting are generally milder for patient with darker or more delicate skin.
In addition to lasers these are a few of the other options available when it comes to treating Asian Acne scarring:
Venus Viva
Venus Viva was FDA approved I the united states in 2015.
The device has 2 hand-pieces; the Diamond hand piece is non ablative and tightens the skin without any downtime. In fact it does not break the skin and clients can return to work immediately. Several sessions are needed.
The second hand piece is the nano-fractional ablative Radiofrequency. Tiny prongs/needles actually penetrate the skin and release radiofrequency energy under the skin (in the dermis). This is where the actually scarring is. There is 2-3 days of redness and occasional peeling.
1-2 sessions over 4 weeks are needed to get significant improvement.

A question I am often asked by patients relates to the efficacy of specific procedures for the elimination of acne scars. The answer is invariably disappointing to them: No one procedure is capable of complete removal of all types of Acne scarring.
Patients often come in and ask for Micro-needling or Subcision or a certain laser to “eliminate” their acne scars.
I spend a substantial portion of my consultation explaining the difference between ice-pick scars, rolling scars, atrophic scars or box-car scars. I then proceed to explain the difference between fillers, lasers and tightening procedure.
Often times a combination of these modalities are needed to improve acne scars.
Deep atrophic scars need to be filled in.
Ice pick scars may need to be excised.
Rolling and Box-car scars need a combination of Subcision and Co2 laser.
My goal is to educate my patient and chose the best and most effective procedure combination for them.
I also make sure that their expectations are realistic. Most patients get 25-30% improvement with one combination treatment.
A second or even third treatment may be needed for even better results.

I often see patients with deep acne scarring who have atrophic, ice pick, box car, and rolling scars.
These types of scarring are difficult to treat. Each type of scar requires a separate modality and special consideration.
Ice pick scars are punched out using different size round punches. The holes are basically excised and the skin surtured back in place. The ensuing scar from the excision is evened off using a CO2 fractional laser (Mixto CO2).
Rolling and atrophic scars are treated with Subcison.
Subcision involves using a small knife under the skin to cut the bands that are pulling the skin down.
The skin is tightened using Ultherapy or Pelleve.
